Work Schedule:             Mon-Fri hours 9:00 AM-5:00 PM; hours may vary occasionally

Salary:                           $50,000/annually

 

***This position is covered by the SSG Fox Suicide Prevention Grant Program, which runs from October 1, 2024 - Sept 30, 2025 with an anticipated renewal annually through Sept 30, 2027.***

 

 Job Summary                                                                                                                                     

 

Clear Path for Veterans, Inc. (CPV or Clear Path) is a community-based organization delivering world-class, holistic programs and services to veterans and their families without condition. The SSG Fox SPGP Navigation Coordinator is responsible for conducting outreach to identify, assess, and engage veterans at risk for suicide, conducting comprehensive intakes and assessments to assess veterans for suicide risk and program eligibility, and connecting eligible veterans to SSG Fox SPGP Peer Mentors for ongoing peer support services. The incumbent of this position may be required to consult with therapists, psychologists, and other health care professionals regarding veterans participating in the SSG Fox SPGP.

 

 Primary Functions                                                                                                                              

 

  1. Conduct initial assessments with veterans to assess suicide risk and SSG Fox SPGP eligibility using validated tools provided by the VA in the Data Collection Tool.
  2. Administer the five required mental health screenings provided by the VA at program intake, at regular intervals (every 120 days), and upon program completion in the Data Collection Tool
  3. Regularly review screening scores with each veteran’s Peer Mentor to measure improvements in mental health, including reduced suicidality, and to inform the veteran’s Individualized Service Plan.
  4. Connect eligible veterans to SSG Fox Peer Mentors to receive Peer Support and resource coordination.
  5. Provide readily accessible resources to all veterans, their families, and caregivers to increase access to suicide prevention services including the Veterans Crisis Line or the 988 Suicide and Crisis Lifeline.
  6. Provide mental health first aid to veterans experiencing immediate crisis while participating in program-related activities and direct them to the VA and other community resources, as necessary.
  7. Timely completion of required paperwork, data entry and reports for CPV’s SSG Fox SPGP.
  8. Assist the Program Manager in ongoing program evaluation activities using VA data dashboards, Salesforce, and mental health screening scores to monitor overall program performance.
  9. Participate in bi-weekly Care Coordination Meetings with the SSG Fox SPGP Team.
  10. Attend monthly SSG Fox SPGP National webinars and meetings.

 Education and Experience Requirements                                                                                           

 

  1. Bachelor’s degree in social work, psychology, public health, or a related field and experience working with veterans with mental health issues.
  2. Candidates for this position must meet the requirements of 38 S.C. 7402(b)(13).
  3. This position requires the ability to work a flexible schedule, including non-traditional hours as necessary to accommodate for special event attendance.

Must be able to lift a maximum of thirty-five (35) pounds. Ability to drive and travel in all types of weather conditions. May sit or stand for long periods of time. Manual dexterity sufficient for constant use of computer keyboards and other office equipment. Vision sufficient for frequent use of a computer monitor, developing and reading paperwork.
