ACTT Housing Specialist (Qualified Professional – QP)

💰 $24.50 per hour

Pyramid Healthcare is seeking a Housing Specialist (Qualified Professional) to join our Assertive Community Treatment Team (ACTT) in Charlotte, NC. This role focuses on helping individuals with serious and persistent mental illness (SPMI) obtain and maintain safe, stable housing while supporting recovery and independence in the community.
This is a community-based role where you will work closely with clients, landlords, and community partners to remove barriers to housing stability.
Key Responsibilities

Assist clients with locating and securing stable housing, including supportive and affordable housing options.

Support clients with housing applications, lease processes, and housing documentation.

Develop relationships with landlords, housing authorities, and community housing programs.

Provide tenancy support and housing stability services to help clients maintain housing.

Assist clients in building independent living skills, including budgeting and maintaining housing.

Work collaboratively with the ACTT multidisciplinary team to integrate housing goals into treatment plans.

Provide community-based services in client homes and community settings.

Maintain accurate documentation and participate in ACTT team meetings and treatment planning.
Qualifications

Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a related field.

Must meet Qualified Professional (QP) status per 10A NCAC 27G.0104.

Experience in housing services, case management, supportive housing, or community mental health preferred.

Experience working with individuals with serious and persistent mental illness (SPMI) preferred.

Valid driver’s license with ability to travel locally for community-based services.
