Join Our Team as a Case Coordinator at Pyramid Healthcare!

Location: York, PA
Schedule: Monday – Friday | 8:00 AM – 4:00 PM
Pay Rate: $16/hour

Pyramid Healthcare in York is on a mission to provide exceptional, client-centered treatment for adults facing Substance Use Disorders. As a recognized leader in addiction treatment in York County and across Pennsylvania, we pride ourselves on our passionate leadership team and dedicated staff. We are excited to announce an opening for a Case Coordinator who will be instrumental in coordinating aftercare appointments, maintaining communication with referral sources, and keeping legal or external agencies informed of our clients' progress.

Key Responsibilities:

Ensure clients have access to the full continuum of care by assessing their needs and facilitating appropriate services.
Coordinate all appointments, aftercare plans, schedules, and meetings.
Arrange Halfway House interviews and placements.
Obtain necessary information and releases from clients, families, referral sources, and other agencies.
Perform other duties as assigned.

Qualifications:

High school diploma or equivalent required.
