Summary: 

We are seeking a dedicated Case Coordinator Manager to oversee our dynamic case coordination team. This role ensures seamless client engagement and coordination with internal treatment teams and external providers. The ideal candidate will have strong leadership skills, excellent communication abilities, and a passion for supporting client recovery journeys.

Key Responsibilities:

  • Supervise and engage case coordinator staff, providing regular oversight and guidance.
  • Develop and maintain staff schedules.
  • Coordinate client engagement, including appointments, aftercare plans, and collaboration with the treatment team.
  • Maintain professional communication with referral sources and outside providers, addressing needs related to UR, legal, and employment.
  • Provide direct case coordination services, including transportation coordination and individual sessions.
  • Assist in developing aftercare plans and facilitating referrals to appropriate providers.
  • Facilitate support groups to enhance clinical services and client engagement.
  • Document all client interactions, family communications, and external contacts in Carelogic within program timelines.
  • Collaborate with the client navigation team to address aftercare needs and support Enhanced Outpatient Sessions (EOS).
  • Conduct performance evaluations and provide ongoing training to case coordinator staff. Participate in corporate training, program development, and professional growth activities. 

Qualifications: 

  • High School Diploma or equivalent required.
  • Experience: 2 years in a drug and alcohol or mental health setting preferred.
  • Previous supervisory experience is highly desirable.
  • Certifications: Current CPR, AED, and First Aid certifications required.
  • Valid driver's license preferred.

Skills and Abilities:

  • Excellent oral and written communication skills.
  • Strong interpersonal and clinical judgment abilities.
  • Effective problem-solving and crisis management skills.
  • Knowledge of federal and state standards, as well as regulatory body requirements.
