Come join our Clinical Team as a Case Manager for our clients in treatment. The Charlotte Hall Drug & Alcohol Residential Treatment Center has served local communities for more than 40 years, providing treatment through structured programs that guide our clients through detox, residential care, medication-assisted treatment, and family education for both men and women. Under a passionate leadership team, Pyramid Healthcare offers comprehensive behavioral healthcare for our clients to help develop the strength they need to overcome life’s challenges and allow clients at all stages of recovery or rehabilitation to reclaim health and well-being.
Hourly Rate: $19-20/hour based on experience
Major Areas of Responsibility:
- Acts as the referral linkage point for clients in developing and implementing a continuing care plan prior to their discharge date.
- Follows up with all clients who leave AMA (Against Medical Advice) within 24 hours to ensure that they are connected to further treatment.
- Acts as a first point of contact for outside tri-county clients admitted into Anchor to connect them with the appropriate care coordinator within 7 days of admission.
- Provides follow-up communication with referring agents prior to discharge.
- Provides intake individual and group education/treatment services in accordance with licensing, certification, & productivity standards.
- Maintains documentation per agency quality assurance procedures and established clinical performance standards.
- Other duties as assigned.
Licensure, Education, & Experience:
- Associates or Bachelor’s degree in Human Services or related field.
- ADT Status for State of Maryland or following certification/licensure: CSC-AD, CAC-AD, LGPC, LGSW, LCPC.
