We are seeking a motivated, organized Case Manager to join our clinical team. In this role, you’ll play a key part in coordinating care, supporting client engagement, and helping individuals successfully transition back into the community.


Position Summary

The Case Manager provides care coordination and case management services for clients and their families in a residential treatment setting. This role supports engagement in treatment through individual and family services, group facilitation, and collaboration with the multidisciplinary team to ensure seamless admissions, treatment planning, and aftercare coordination.


Schedule & Pay

  • Schedule: Monday – Friday
  • Status: Full-Time
  • Pay: $19 – $21 per hour

Key Responsibilities

  • Coordinate client schedules, appointments, and treatment-related activities in collaboration with the clinical team
  • Maintain consistent communication with referral sources and external providers, ensuring continuity of care
  • Assist with the admissions process, ensuring all required documentation and consents are completed and properly recorded
  • Provide client and family orientation in collaboration with the primary therapist
  • Deliver individual and family case management services to support treatment engagement and address external needs
  • Coordinate transportation and connect clients with community resources
  • Facilitate psychoeducational and skills-based groups
  • Develop and coordinate comprehensive aftercare plans with the treatment team and business development staff
  • Ensure all documentation, including aftercare plans, is completed in CareLogic in accordance with program standards
  • Document all client, family, and provider interactions accurately and in a timely manner
  • Maintain confidentiality in accordance with HIPAA and organizational policies
  • Participate in required trainings and ongoing professional development

Qualifications

Education & Experience:

  • Bachelor’s degree in a human services field + 2 years of relevant direct care experience
    OR
  • Bachelor’s degree in a non-human services field + 4 years of relevant direct care experience

Skills & Abilities:

  • Strong organizational and time-management skills
  • Excellent communication and interpersonal abilities
  • Ability to collaborate effectively within a multidisciplinary team
  • Knowledge of behavioral health, substance use treatment, and community resources preferred
  • Ability to maintain professional boundaries and confidentiality
