Location: Pyramid Healthcare, Prince George County Facility 3000 Lottsford Vista Road, Bowie, MD

About Us: Come join our Clinical Team as a Case Manager, where you will work with clients in treatment. Pyramid Healthcare has been serving local communities for over 40 years through structured programs that guide our clients through detox, residential care, medication-assisted treatment, and family education for both men and women. Under a passionate leadership team, we offer comprehensive behavioral healthcare to help clients develop the strength they need to overcome life’s challenges and reclaim their health and well-being.

Hourly Rate: $19 - $20/hour based on experience

Major Areas of Responsibility:

  • Act as the referral linkage point for clients in developing and implementing a continuing care plan prior to their discharge date.
  • Follow up with all clients who leave Against Medical Advice (AMA) within 24 hours to ensure they are connected to further treatment.
  • Serve as the first point of contact for outside tri-county clients admitted into our program, connecting them with the appropriate care coordinator within 7 days of admission.
  • Provide follow-up communication with referring agents prior to discharge.
  • Deliver intake individual and group education/treatment services in accordance with licensing, certification, and productivity standards.
  • Maintain documentation per agency quality assurance procedures and established clinical performance standards.
  • Perform other duties as assigned.

Licensure, Education, & Experience Requirements:

  • Associate’s or Bachelor’s degree in Human Services or a related field required.
  • ADT status for the State of Maryland or relevant certification/licensure: CSC-AD, CAC-AD, LGPC, LGSW, LCPC.

Schedule: Monday - Friday: 12:00 PM - 8:00 PM
