The Role
The Social Intake Coordinator manages Regional Center Social Assessment referrals from intake through completion and submission, serving as the primary point of contact for individuals and families by coordinating outreach, scheduling, assessments, and required documentation in Salesforce. This dedicated role partners with Regional Center and internal teams to ensure assessments are completed in a timely manner and associated fees are accurately processed for billing. The role does not provide ongoing service coordination or manage a service caseload.
Primary Responsibilities
- Manage Social Assessment referrals from intake through completion and submission, conducting timely outreach and follow-up to complete an average of three assessments per day.
- Coordinate and schedule assessment appointments, serving as the primary point of contact for individuals and families throughout the assessment process.
- Conduct structured Social Assessments and gather required social, developmental, behavioral, and environmental information while building rapport, providing excellent customer service, and effectively de-escalating concerns or challenging interactions as needed.
- Complete and maintain accurate assessment documentation and Salesforce records, meeting established quality, accuracy, and turnaround expectations.
- Partner with Regional Center and internal teams to address workflow needs, resolve issues, and ensure completed assessments are appropriately tracked and ready for fee processing.
- Support assessment and workflow needs across assigned territories, adapting to changing priorities and assisting other service areas as needed to support team and organizational goals.
This is a hybrid position, coming into the El Segundo office 1x per month.
Work Schedule: Business hours are 8:00 a.m. to 6:00 p.m. Pacific Time. Depending on the position and business needs, employees may be required to work any scheduled shift within these operating hours.
What You Bring to the Table:
Qualifications
- Bachelor’s degree required.
- Minimum 3 years of experience in person-centered practices, developing care plans, or a related service-delivery environment strongly preferred.
- Experience supporting individuals with intellectual and developmental disabilities or working within social recreation or related services preferred.
- Bilingual Spanish preferred.
