The Role:

 

The Care Connect Specialist manages member and caregiver services within the CareConnect service line under Community Supports. This role supports members from referral through active services while helping facilitate successful caregiver matches and maintain continuity of care.

The Care Connect Specialist serves as a primary point of contact for members and caregivers, providing proactive outreach, resolving service needs, maintaining accurate records, and partnering across teams to improve the overall CareConnect experience.

Primary Responsibilities

  • Support members through the referral and admission process, maintain the pending provider roster, complete required documentation, and help facilitate successful caregiver matches.
  • Maintain the caregiver registry, including caregiver availability, activity status, documentation, engagement, and appropriate additions or removals.
  • Serve as a primary point of contact for members and caregivers, responding to calls, texts, chats, and emails and working toward timely, one-call resolution whenever possible.
  • Proactively address caregiver responsiveness, scheduling, and engagement concerns while identifying opportunities to reduce referral-to-service timelines and improve authorization utilization.
  • Maintain accurate member, caregiver, and activity information in Salesforce while partnering across teams to support service continuity and HIPAA compliance.

 

This is a hybrid position, coming into the El Segundo office 1x per month. 

Work Schedule: Business hours are 8:00 a.m. to 6:00 p.m. Pacific Time. Depending on the position and business needs, employees may be required to work any scheduled shift within these operating hours.

 

What You Bring to the Table:

Qualifications

  • Bachelor’s degree preferred, or an equivalent combination of education, training, and experience.
  • 1–3 years of client services, fulfillment operations, or customer service experience.
  • Experience supporting clients or customers through a multi-step service or fulfillment process.
  • Experience working in a fast-paced environment while managing multiple priorities and deadlines.
  • Bilingual Spanish preferred.

The expected California Pay Range for this position:

$26.04 - $28.97 USD
