💻 The Role
We are seeking a dynamic and compassionate individual to join us as a QA Nurse. This role will report into the Clinical Audit Manager. This is a Full-Time Remote position based in the United States. In this role, you’ll be responsible for reviewing clinical documentation to ensure it is complete, compliant, and meets both internal standards and external regulatory requirements (e.g., CMS, CHAP, etc.). You’ll be an essential partner in maintaining the integrity of our clinical records and helping to ensure we deliver safe, high-quality care.
The Requirements
- Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Pennsylvania
- 2+ years of clinical experience, ideally in home health, hospice, or a regulated healthcare setting
- Prior experience with QA, auditing, or clinical documentation review
- Working knowledge of CMS and/or Medicaid regulations
- Attention to detail, a methodical, thorough work style, and strong communication skills
- Ability to work independently while staying aligned with broader QA and compliance processes
- Familiarity with EMR systems and comfort working with digital documentation and spreadsheets
- Experience with CHAP or other accreditation standards is preferred
- Experience working in a multi-state home health agency is preferred
Responsibilities
- Review clinical documentation for accuracy, completeness, and compliance with regulations.
- Identify errors or missing information and coordinate corrections with clinical staff.
- Ensure timely QA of documents for billing, audits, and regulatory deadlines.
- Support education for nurses and caregivers on documentation standards.
- Maintain organized logs of QA reviews, corrections, and outstanding items.
- Participate in internal audits and assist with survey and accreditation preparations.
- Escalate documentation or compliance concerns to the QA Manager as needed.

This is a Full-Time role with the anticipated annual compensation of $60,000 - $66,000 base + bonus potential and benefits.
