The Role
We are seeking a dynamic and compassionate individual to join us as a Care Manager This role will report into the Care Experience Manager in Indiana. This is a Full-Time Hybrid position based in Fort Wayne, IN. In this role you will provide guidance and support to clients and caregivers on the Health & Wellness and Pathways Waiver Programs.
The Care Manager is responsible for assessing the client’s needs, developing a personalized service plan, and creating a care schedule for both the client and caregiver. They also provide ongoing support to the primary family caregiver and client, regularly monitoring progress and making schedule adjustments as needed.
Key Responsibilities:
- Manage and oversee client care plans in alignment with physician instructions, including in-home visits.
- Provide support through in-home, virtual, or phone-based services using HIPAA-compliant communication platforms.
- Document all caregiver and client communications received through approved electronic platforms (e.g., app or email) to ensure accurate and timely sharing of information with Case Managers.
- Partner with caregivers and clients to develop, implement, and update individualized service plans as needs evolve.
- Submit referrals to help families to access local resources/services based on individual needs (such as housing, food insecurity, medical care, etc)
- Collaborate with the remote care team to provide wraparound support for clients, including submitting incident reports and communicating incidents to the Case Manager as needed
The Requirements:
- An Associate’s or Bachelor’s Degree in social work, human services, or related field
- Strong observational, problem-solving, and reporting skills
- Ability to follow instructions while exercising independent judgment
- Active listening skills with empathy and compassion
- Reliable transportation and a valid driver’s license in the state of Indiana
- Experience working with families in a home setting is preferred
- Familiarity of the local client base and available resource options preferred
- Background in Social Work or Case Management field is preferred
- Bilingual in Spanish is preferred

- This is a Full-Time role with an estimated annual salary range of $55,000 - $70,000 base salary + bonus and benefits.
