The Role

We're looking for a passionate Registered Nurse to join our team as an RN Case Manager. This role will report into the Director of Nursing in Florida. This is a full-time hybrid opportunity based in Miami, FL. You'll help develop plans of care and deliver high-quality skilled nursing care in the home health setting.

Key Responsibilities: 

  • Provide patient care and coordination. Manage a diverse caseload, ensuring quality patient care to our pediatric patients.
  • Craft and implement treatment plans. You'll be involved in developing and implementing care plans, providing care in-home, and oversee patient progress.
  • Collaborate with families. You'll be working with families to provide high-quality, compassionate care, and provide coaching support.
  • Utilize digital health as a strength. You'll establish and maintain effective communication with our patients' PCPs and specialists. You'll ensure all documentation meets regulatory standards. You'll be leveraging technology to scale the impact of your time, communicate with the care team, and interact with families.
  • Partner with operations, payers, technology, and product teams across Abby Care. You'll be a critical voice for working cross-functionally to help our children meet great health outcomes and furthering our mission.

The Requirements:

  • Must be licensed in the state of Florida and currently reside in the Miami area
  • 2 years of experience as a Registered Nurse in a clinical setting; 1 year of home health experience preferred.
  • Case management/supervisor experience and pediatric/adult care experience preferred.
  • Knowledge of Medicaid paid family caregiving program and CNA/HHA scope of practice preferred (training provided if needed).
  • Reliable transportation, valid driver's license, and current auto insurance required.
  • Ability to work flexible hours, travel locally, and provide on-call service to patients/families.
  • Knowledge of OASIS preferred (training provided if needed).
