The Role

We're looking for a passionate Registered Nurse to join our team as an IHSS RN Case Manager. This role will report into the Director of Nursing in Colorado. This is a Full-Time Hybrid opportunity based in Denver, CO. The IHSS RN Case Manager at Abby Care is responsible for overseeing the comprehensive care and coordination of services for patients within their caseload. Provide supervision and oversight of all IHSS cases including; patients, their families, and personal care providers. 

Key Responsibilities: 

  • Administer skills validation tests to ensure competency, while also provide guidance to attendant staff on challenging cases and potentially hazardous situations.
  • Verify and document attendant skills and competency to perform IHSS and basic consumer safety procedures.
  • Consult with the client, authorized representative or attendant in the event a medical issue arises.
  • Investigate complaints and critical incidents within 10 working days.
  • Verify that the attendant is following all services set forth on the IHSS care plan.
  • Review the care plan and the Physician Attestation form upon initial enrollment, following any change in condition, and upon the request of the client, the authorized representative, or the case manager.
  • Provide in-home supervision for the client as agreed upon by the client or their authorized representative.

The Requirements:

  • An Associate's Degree or A Baccalaureate School of Nursing Degree.
  • Current and valid license as a Registered Nurse in the state of Colorado.
  • 2 years of experience as a Registered Nurse in a clinical setting; 1 year of home health experience preferred.
  • Case management/supervisor experience and pediatric/adult care experience preferred.
  • Knowledge of Medicaid paid family caregiving program and HHA scope of practice preferred (training provided if needed).
  • Reliable transportation, valid driver's license, and current auto insurance required.
  • Ability to work flexible hours, travel locally, and provide on-call service to patients/families.
  • Knowledge of OASIS preferred (training provided if needed).
