AbilityPath is hiring for a Help Me Grow Care Coordinator!
The HMG Coordinator works as the primary resource for families with questions regarding child development and supports early screening for young children and linking families with resources with San Mateo County.
This is a full time position, hybrid role, and candidates are required to be Bilingual in English/Spanish or other high-frequency languages in San Mateo County

ABOUT THE ROLE:

- Provide information and resources to parents of children 0-5 about child development
- Promote Ages & Stages Questionnaire (ASQ) developmental screenings to parents
- Follow HMG SMC Call Center protocols for answering calls/text/emails/web inquiries, triage, follow-up, mandatory reporting
- Promote and process ASQ online developmental screenings
- Provide short-term care coordination to families with children between 0-5 years at risk for a developmental delay
- Make appropriate referrals for community and developmental services
- Make and receive warm referrals to the Family Resource Center team using Warm Referral Protocols
- Enter all family and provider data into data collection system in a timely manner according to the workflow protocol.
- Regularly identify families experiencing barriers within the following systems
- Develop collaborative relationships with partners to coordinate and implement Tu y Yo parent/child program
- Through outreach, develop collaborative relationships with community agencies, project lead agencies and partners to support the HMG SMC system
- Proactively research and participate in provider and family outreach events (e.g. resource fairs, meetings, events, etc.) to promote the HMG system and Call Center.
- Participate in staff meetings, conferences, reflective practice meetings, and workshops regularly to further develop professional skills and to improve the understanding of systems and current best practices for working with children and families
- Work both independently and as part of a team in an evolving and rapidly changing work environment
- Other duties as assigned
EDUCATION AND WORK EXPERIENCE:

- Bachelor’s Degree required in child-development,
social work, psychology or related field - Minimum of 3 years of relevant work experience
with families and children birth through five
- Bilingual in English/Spanish or other high-frequency languages in San Mateo County such as Portuguese, Mandarin, Cantonese, or other Asian/Pacific Islander languages.
HIRING REQUIREMENTS:
- Valid California Driver’s License/ID
- Clean Driving Record (if applicable)
- Proof of negative TB test results
- Verification of occupational physical clearance
- Pass a criminal fingerprint clearance
- 18 years of age or job permit clearance
PHYSICAL ACTIVITY:
