This role guides multi-site clinic performance through strong relationships, practical leadership, and consistent follow-through.

The Director of Operations partners closely with the Regional Administrator to ensure alignment on regional goals, financial performance, provider support/recruitment, and team development. • Support daily clinic operations including access, scheduling alignment, staffing support, and workflow consistency.
  • Serve as the first level of escalation for operational challenges within the region.
  • Collaborate with providers to support sustainable productivity, onboarding, and engagement.
  • Monitor clinic performance trends including access metrics, patient experience, staffing efficiency, and financial indicators.
  • Foster collaboration across providers, clinical support teams, and regional support departments.
  • Represent Praxis Health within the Southwest Valley community and strengthen local partnerships.
  • Support implementation of value-based care initiatives and quality improvement efforts.
  • Leadership Expectations

The Director of Operations leads with clarity, consistency, and collaboration. This role requires an action-oriented mindset balanced with relational leadership. Success in SWV depends on earning trust, communicating transparently, and bringing teams together around shared goals.

This leader maintains strong working relationships across all levels of the organization, including providers, clinic support staff, clinic Managers, community partners, and the broader Praxis Health leadership team.

This role requires someone who can balance urgency with patience and move initiatives forward without losing the human element.

The Right Candidate

  • A relational leader who earns trust quickly
  • Clear and consistent in communication
  • Comfortable using data to inform conversations without leading with spreadsheets
  • Accountable, steady, and willing to step in when needed
  • Focused on practical progress over perfection

You understand that high performance is not about pressure, it is about clarity, partnership, and consistent follow-through.

Form CC-305
OMB Control Number 1250-0005
Expires 04/30/2026

Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
