The Opportunity
As a Lead Community Health Worker, you balance a patient caseload with hands-on coaching for the CHW team. You precept new hires, field real-time workflow questions, coach less experienced CHWs, and help interview future teammates to keep quality high. Positioned on the front line, you also test new tools and processes, channeling practical feedback to leadership so Akido’s care keeps getting better.
What You Do
- Perform all duties outlined in the CHW job description (e.g. caseload management, care-plan creation, patient coaching, appointment accompaniment, SDoH referrals, thorough documentation, and outreach/enrollment for Akido’s ECM program).
- Primary preceptor for new hires: host each new CHW for ~1.5 weeks of shadowing during their 3-week onboarding; model best practices in the field, EHR documentation, and patient engagement.
- Serve as the go-to resource for less-experienced CHWs—answering real-time questions about workflows (e.g., General Relief forms, housing referrals, benefits applications) and troubleshooting barriers.
- Provide “ride-along” coaching sessions and real-time feedback to reinforce trauma-informed, patient centered care.
- Serve as early adopters and champions of new technologies and workflows. Give feedback to management as part of the implementation process.
- Participate in onsite interviews for CHW candidates whenever logistically feasible; assess cultural fit, field readiness, and empathy-driven problem solving.

Compensation
$33/hr
Salary range
$68,640 - $68,640 USD
