About the Role:
As the Director of Case Management, you will lead and oversee the delivery of exceptional person-centered services. In this role, you’ll guide a team of case managers, ensuring individualized support plans are co-created and implemented effectively. Your leadership will empower individuals and their support teams to navigate services that enhance lives, while also driving compliance, efficiency, and innovation across the department.
This role is part of Aspire Living & Learning’s exciting opportunity to partner with the State of Vermont under a new contract. We expect to learn the outcome of this contract in January 2025. In preparation, we are actively identifying talented individuals to join our team and help us deliver high-quality services to individuals in Vermont. We plan to start reaching out to candidates in early to mid-January to initiate the hiring process and build our team structure—anticipating a successful partnership with the state.
What You’ll Do:
- Provide strategic leadership and supervision for a team of case managers to ensure the highest quality of services.
- Champion person-centered planning processes that reflect individual strengths, preferences, and goals.
- Build and maintain positive relationships with individuals, families, guardians, and service providers.
- Oversee service planning, implementation, and monitoring to ensure satisfaction, compliance, and overall well-being.
- Ensure adherence to state and federal regulations, including Medicaid requirements, as well as organizational policies and procedures.
- Drive continuous improvement initiatives to enhance the efficiency and effectiveness of service delivery.
- Facilitate professional development opportunities for your team to foster growth and knowledge in person-centered practices.
- Monitor and manage case management metrics, ensuring accountability and measurable success.
- Collaborate with internal and external stakeholders to optimize service coordination and resource allocation.
What You Bring:
Education & Experience:
- Bachelor’s degree in a related field required; advanced degree preferred.
- Minimum of five years of experience in human services, with at least two years in a leadership role.
- Proven expertise in person-centered practices and regulatory compliance in developmental and intellectual disability services.
Leadership & Skills:
- Strong interpersonal, organizational, and problem-solving skills.
- Demonstrated ability to lead and motivate diverse teams.
- Deep understanding of trauma-informed care and evidence-based practices.
- Proficiency in Microsoft Office, data analysis tools, and electronic health records.

Working Conditions:
- Ability to travel between service locations as required.
- Physical capacity to handle job tasks, including occasional lifting up to 50 pounds.
- Participation in an on-call rotation to provide leadership support after hours.
