About the Role: 

As a Case Manager, you’ll play a pivotal role in helping individuals live fulfilling, meaningful, and independent lives. Through person-centered service planning, you will partner with individuals, families, and service providers to identify goals, dreams, and support needs. Your work will create lasting impact by connecting individuals with essential services and resources while ensuring regulatory compliance and promoting continuous improvement.

This role is part of Aspire Living & Learning’s exciting opportunity to partner with the State of Vermont under a new contract. We expect to learn the outcome of this contract in January 2025. In preparation, we are actively identifying talented individuals to join our team and help us deliver high-quality services to individuals in Vermont. We plan to start reaching out to candidates in early to mid-January to initiate the hiring process and build our team structure—anticipating a successful partnership with the state.

 What You’ll Do: 

  • Build strong, collaborative relationships with individuals, families, guardians, and service providers. 
  • Facilitate person-centered service planning to align with each individual's unique strengths, preferences, and goals. 
  • Provide information on services, resources, and self-direction options to empower informed decisions. 
  • Monitor and support service plan implementation while ensuring satisfaction and well-being. 
  • Maintain compliance with all state, federal, and organizational regulations. 
  • Advocate for individual rights, choice, and control within systems of support. 
  • Participate in training and contribute to a positive workplace environment. 

What You Bring: 

  • Education & Experience: 
    • Bachelor’s degree in a related field preferred or equivalent experience. 
    • Minimum of two years in the human services field. 
  • Skills & Competencies: 
    • Strong interpersonal, organizational, and communication skills. 
    • Understanding of person-centered principles, trauma-informed care, and evidence-based practices. 
    • Familiarity with Medicaid regulations and human services systems. 
    • Proficiency in Microsoft Office and electronic health record systems. 
    • Person-Centered Competencies: 
      • A strengths-based, culturally informed, and whole-person-focused perspective. 
      • Expertise in fostering connections inside and outside formal systems. 
      • Commitment to individual rights, choice, and control. 
      • Proven skills in teamwork, facilitation, and coordination. 
      • Strong experience in plan documentation, implementation, and monitoring.

Working Conditions: 

  • Ability to travel as required with reliable transportation. 
  • Participation in an on-call rotation to provide after-hours support.
