Job Purpose: 

The Outreach and Development Manager- Community Companion Home at Aspire Living & Learning plays a vital role in the recruitment and on-boarding of new Community Companion Home licensees, with the purpose of expanding services to individuals in need of Community Companion Home (CCH) services for the organization.  The CCH Outreach and Development Manager will be familiar with and carry out support, training, and licensing process in adherence with Connecticut Department of Developmental Services (DDS) regulation and guidelines, as well as agency policies.  

 

Key Responsibilities: 

  • Recruit and on-board new licensees in adherence with CT DDS regulations, guidelines and agency policy. 
  • Develop recruiting and marketing plans that will meet CCH development goals setDevelop plans will be maintained and updated electronically.  
  • Complete a monthly report summarizing outreach and marketing activities completed, including the result of those activities, upcoming calendar of events and report out on CCH programming including census in relation to development goals.   
  • Work collaboratively with Marketing Department for outreach and materials.  
  • Maintain appropriate records and electronic filing to include records of training, CCH agreements and individual-specific records as outlined by agency policy. 
  • Oversees all licensing inspections including quality assurance visits for all licenseesSupports/assists with any plans of corrections. 
  • Liaison with DDS in the area of CCH programs – CCH leadership, Licensing and Case Management.   
  • Maintenance of licensing requirements for all CCH homes per state regulations. 
  • Development, evaluation, and implementation of training for all licensees per state regulation and agency policy. 
  • Responsible for coordination of services for individuals supported in our CCH program.   
  • Respectful and supportive engagement with individuals, families, licensees and funding source representatives. 
  • Familiarity with and implementation of agency priorities. 
  • Active participation in departmental and agency-level meetings and events. 
  • Supervision and oversight of team members. 
  • Makes monthly visits to CCH providers/individual residents and interfaces with DDS Case Mangers to coordinate care and support for assigned caseload. 
  • Participates in individual care planning meetings (ISP) and writes quarterly/annual reports as requiredFollows up with DDS Case Manager/Licensees to ensure scheduled within timeframe 
  • Completes home study evaluations and recertifications and attends all licensing reviews as required by DDS.   
  • Ensures timely completion of Licensing Applications and pre-licensing requirements, Licensee Agreements, Cost of Care and other required documentation. 
  • Makes referrals for and coordinates services for supported individuals as needed and communicates effectively with all parties involved. 
  • Coordinates application process for CCH programs, matching referrals to licensee homes based on the preferences of both parties. 
  • Ongoing collaboration with Aspire Adult Services Team and various departments within the agency to stay current with and contribute to agency priorities. 
  • Responds to internal and external audits in a timely fashion and actively engages in quality improvement processes. 
  • Cultivate a positive and productive workplace atmosphere to enhance team morale and productivity. 
  • Participate in and complete training sessions as delegated by management to enhance skills and knowledge relevant to the role. 
  • Conduct training as required and assigned.  
  • Perform any additional job responsibilities assigned by supervisors or team leaders to support the organization's smooth operation. 
  • Accept reassignment as required. 

 

 

Qualifications: 

  • Bachelor’s degree in human services, social work, psychology, counseling, or a related fieldExperience may be considered in lieu of degree. 
  • Strong interpersonal, organizational, and communication skills. 
  • Strong computer skills. 
  • Valid driver’s license and safe driving recordThis position requires use of personal vehicle.  
  • Reliable transportation and willingness to travel to sites throughout CT. 
  • 18 years or older. 
  • Must have satisfactory background checks  
  • Valid driver’s license and safe driving record 

 

Working Conditions: 

  • Work will be done in an office setting and on the road visiting program as required to fulfill their job duties.  
  • May be required to travel including infrequent day and/or overnight travel  
  • Some holiday work required to meet on call schedule as needed.  
  • Lift/carry up to 10 pounds; speaking; standing; sitting for long periods of time; reaching; bending; walking; reaching with arms and hands, balancing; using fingers to handle or feel.  Vision abilities required by this job include close vision. 
  • This position requires the routine use of standard office equipment such as computers, phones, copiers, fax machines, audio/visual equipment, etc.

Salary- Starting at $62,000/YR
