Aspire Living & Learning is currently seeking a License Practical Nurse (LPN) to play a vital role in overseeing the quality of health and nursing care for individuals served by Aspire Living & Learning, Inc. This involves understanding the particular needs of each individual within the program; making sure our employees and health care providers are attending to all needs identified and complying with state regulations and Aspire policies and procedures; training and educating all parties on essential health related matters; and providing on-going consultation and support to the individuals, staff, providers, coordinators and family/guardians. 

Schedule: Monday, Tuesday, Thursday, and Friday 7am-3pm

Location: Trumbull, CT

Qualifications:

  • Excellent verbal and written communication skills.   
  • Must have demonstrated computer literacy.  
  • 18 years of age or older.  
  • Excellent organizational skills and attention to detail.  
  • Basic understanding of clerical procedures and system.  
  • Ability to pass and maintain required trainings.  
  • Must possess an active nursing license in the state in which you’re working. 
  • Must have satisfactory background checks  
  • Valid driver’s license and safe driving record 

Key Responsibilities: 

Ensure individuals medical needs are met; 

  • Participate in the delivery of health care services to the individuals as well as being responsible for the direct care needs of the supported individuals as deemed appropriate per staff in charge.  
  • Participate in the IEP/ISP process and implementation, attend team meetings and advocate for the individual promoting dignity and respect.   
  • Evaluates episodic health needs through nursing assessment and continuous observation. 

Communication;

  • Communicate health issues to staff and update the individuals’ families/guardians in matters including hospitalizations, rehabilitation support such as physical/occupational therapy.  
  • Participate in any medical intervention process until resolution is achieved.   
  • Provide relevant health-related training to staff, families and individuals as needed and assigned.    
  • Initiate referrals for needed medical services including scheduling, hospital safeguarding, advocacy and follow-up care. 

Administrative functions

  • Maintain proper and timely documentation as needed and assigned by supervisor(s), using the electronic record.   
  • Order medications and maintain medical supplies, materials and equipment as needed.  
  • Dispense medications as required using state guidelines and Aspire’s policies and procedures.   
  • Cultivate a positive and productive workplace atmosphere to enhance team morale and productivity. 
  • Participate in and complete training sessions as delegated by management to enhance skills and knowledge relevant to the role. 
  • Conduct training as required and assigned.  
  • Perform any additional job responsibilities assigned by supervisors or team leaders to support the organization's smooth operation. 
  • Accept reassignment as required.

Pay: $40,000-$53,000
