IM Care Coordinator
The IM Care Coordinator provides essential support to all Internal Medicine Services, primarily on weekends and as needed for service gaps. This role ensures seamless coordination of patient care, client communication, appointment management, and procedural support across all Internal Medicine areas. The IM Care Coordinator also performs post-procedure callbacks to ensure continuity of care. This role is critical to improving IM Service workflow, maintaining communication standards, and enhancing client and referring veterinarian (RDVM) satisfaction.
Shifts:
- Sunday – Wednesday (9a – 6:30p)
- Wednesday – Saturday (9a - 6:30p)
Skills Required:
- Minimum of 2 years Veterinary/Human Referral Coordinator experience
- Minimum of 2 years Veterinary Clerical experience
- Minimum of 2 years customer service in a professional setting
- Excellent written skills, well organized and excellent task management skills
- Ability to interact with all levels of staff, clients and veterinarian practices
- Ability to work collaboratively, and independently in a fast-paced, demanding environment, handling multiple tasks with competing deadlines
- Must be able to work holidays and weekends
- Must have ability to remain composed in stressful and demanding situations
- Must exhibit good organizational, task management skills and communication skills
- Must be able to follow detailed protocols and instructions
- Must neatly and correctly fill out paperwork as necessary
Specific Duties and Responsibilities:
- Serve as primary liaison between IM, other services, clients, and DVMs
- Answer incoming calls, handle inquiries and direct them appropriately
- Schedule appointments and procedures based on client or doctor requests
- Confirm appointments, manage and maintain waitlist
- Obtain and attach medical records for inpatients and upcoming appointments
- Review and audit upcoming appointments to ensure all requirements are met
- Respond to doctor requests, rescheduling appointments, and handling time-sensitive follow-ups
- Follow up with clients after hospitalization or procedure discharge
- Keep RDVMs informed and updated as needed
- Assist doctors in submitting prescriptions or calling pharmacies for approval
- Send medical records to owners and RDVM upon request
- Assist with presenting estimates; support clients in accessing financial assistance when applicable
- Must be able to work with sensitive information and maintain confidentiality
- Responsible for front desk responsibilities such as, but not limited to client check-in and check-out which includes patient registration, admission and discharge
- Other duties as deemed necessary
