Hospice RN - Hospital Liaison

As a Hospital Liaison, you will serve as the onsite hospice representative within designated hospitals, building relationships with case managers, physicians, and care teams to identify patients who may benefit from hospice care. You will educate patients and families, perform clinical evaluations, and, when appropriate, complete the admission process to ensure a seamless transition to services.
Overview
The Hospital Liaison serves as an RN representative of Arc Hospice & Palliative Care, working onsite in designated hospital(s) to identify and support patients appropriate for hospice services. This hospital-based role focuses on relationship-building with case managers, discharge planners, and medical staff to facilitate timely hospice referrals. The liaison educates patients and families, performs clinical evaluations, and may complete the hospice admission process. Strong customer service, sales acumen, and clinical expertise are critical to success in this role.
Key Responsibilities
- Serve as the onsite representative of Arc Hospice & Palliative Care within assigned hospital(s).
- Develop and maintain strong working relationships with case managers, discharge planners, physicians, and hospital staff.
- Identify and evaluate patients who may benefit from hospice services through chart reviews and collaboration with care teams.
- Meet with patients and families to review hospice philosophy, explain services, answer questions, and obtain signed consents for care.
- Educate patients and families about hospice philosophy, services, and eligibility.
- Facilitate hospice admissions including completing clinical evaluations, consents, and required documentation.
- Provide timely communication to the intake, admissions, and clinical teams to support seamless transitions to care.
- Maintain detailed, accurate documentation in accordance with clinical and regulatory standards.
- Participate in daily hospital rounds or multidisciplinary team meetings as appropriate.
- Monitor and report on referral trends, barriers to access, and opportunities for process improvement.
- Uphold HIPAA and all applicable clinical, ethical, and regulatory standards.
- Support the mission of Arc Hospice & Palliative Care to deliver high-quality, person-centered care.
- Perform other duties as assigned.
Qualifications
- Graduate of an accredited nursing program; Associate’s or Bachelor’s degree in Nursing preferred.
- Current RN license in the practicing state or valid multistate compact license.
- Minimum of two (2) years of hospice, palliative care, or hospital-based nursing experience required.
- Strong interpersonal, communication, and relationship-building skills.
- Proven ability to educate and support patients and families during emotionally sensitive situations.
- Highly organized, self-directed, and adaptable to changing priorities in hospital settings.
- Proficient with Microsoft Office and electronic medical record systems.
- Valid driver’s license and reliable transportation required for inter-facility travel.
- Demonstrated empathy, sensitivity, and ability to support patients and families from diverse backgrounds.

The starting compensation for this position is shown below. Arc Hospice is committed to transparency and equity in all compensation practices. The rate displayed reflects the starting hourly or salaried pay for the role and does not represent a range.
Compensation
$87,360 - $87,360 USD
