We are seeking a Full-Time Hospice RN – Admissions to serve patients across Navarro County. As the first point of clinical contact for new patients, the Admissions Nurse completes evaluations, facilitates consents, and ensures a seamless transition into hospice services with personalized, compassionate care aligned to each patient’s goals and needs.
Overview
The Hospice RN - Admissions is responsible for completing clinical evaluations, consents, and admission visits for hospice patients. This role ensures timely initiation of services, compassionate communication with patients and families, and seamless collaboration with the clinical and admissions teams.
Key Responsibilities
- Conduct clinical evaluations of patients referred for hospice, assessing eligibility and appropriateness per Medicare Conditions of Participation and organizational guidelines.
- Meet with patients and families to explain hospice services, review consents, answer questions, and guide them through the admission process.
- Collaborate with intake, admissions, and clinical teams to ensure seamless onboarding and timely initiation of hospice services.
- Coordinate with attending physicians, hospital discharge planners, and referring providers to obtain orders and ensure continuity of care.
- Complete and document the admission visit, including initial assessments, medication reconciliation, and initiation of the hospice plan of care.
- Provide emotional support to patients and families, addressing concerns and setting expectations for hospice services.
- Ensure accurate, timely documentation to support clinical, regulatory, and billing compliance.
- Maintain current knowledge of hospice eligibility, services, and regulations, and serve as an educational resource to internal and external partners.
- Provide after-hours or on-call admission support as needed.
- Supports clinical team with routine/symptom follow up visits as needed.
- Perform other duties as assigned.
Qualifications
- Graduate of an accredited nursing program; Associate’s or Bachelor’s degree preferred.
- Current RN license in practicing state or valid multistate compact license; ability to practice in all states of operation.
- Minimum of one (1) year of hospice, palliative care, home health, or hospital-based nursing experience preferred.
- Strong clinical assessment, communication, and documentation skills.
- Ability to compassionately educate and guide patients and families through emotionally sensitive conversations.
- Familiarity with hospice regulations, eligibility, and Medicare Conditions of Participation.
- Valid driver’s license and reliable transportation required.
- Strong communication, critical thinking, and documentation skills.

The starting compensation for this position is shown below. Arc Hospice is committed to transparency and equity in all compensation practices. The rate displayed reflects the starting hourly or salaried pay for the role and does not represent a range.
Compensation
$73,000 - $73,000 USD
