- Position: Hospice Social Worker (MSW)
- Schedule: Full-time; Monday through Friday with on-call participation as needed
- Coverage: Broward County service area
Role Summary
As a Hospice Social Worker at Arc Hospice & Palliative Care, you will provide emotional support, counseling, and resource navigation to patients and families navigating serious illness and end of life. Working as part of the interdisciplinary team, this role supports compassionate, person-centered care aligned with patient goals, values, and regulatory requirements.
Overview
The Social Worker provides psychosocial support, counseling, and care coordination to hospice patients, families, and caregivers. This role collaborates closely with nursing, medical, and spiritual care staff to develop and implement individualized plans of care. The Social Worker also supports bereavement services and may provide supervision to other social work staff in accordance with licensing requirements.
Key Responsibilities
- Provide psychosocial assessments, counseling, and emotional support to patients and families facing terminal illness and grief
- Collaborate with the interdisciplinary team to develop and update individualized plans of care
- Assist patients and families with advance directives, resource navigation, and long-term planning
- Participate in Interdisciplinary Group meetings and care conferences
- Document all services accurately and timely in the electronic medical record
- Provide crisis intervention during acute emotional or practical challenges
- Participate in shared after-hours or on-call rotation as assigned
- Facilitate access to community resources such as transportation, food, housing, and caregiver support
- Support bereavement services including outreach and counseling
- Comply with HIPAA, CMS Conditions of Participation, and state licensure standards
- Provide culturally sensitive support to diverse patients and families
- Perform other duties as assigned
- Supervision (If Applicable): Licensed Clinical Social Workers may provide supervision to Master’s-level social workers or interns in accordance with applicable state licensing board requirements and hospice policy.
Qualifications
- Master’s degree in social work from a CSWE-accredited program
- Current active social work license in the state of practice with eligibility to supervise if applicable
- Minimum of one (1) year of healthcare, hospice, or related experience preferred
- Hospice or palliative care experience strongly preferred
- Knowledge of Medicare Conditions of Participation related to psychosocial care
- Experience with EMR systems and clinical documentation standards
- Strong interpersonal, communication, and documentation skills
- Ability to work independently while collaborating within an interdisciplinary team
- Valid driver’s license and reliable transportation

The compensation range for this position is shown below and reflects the expected pay range for the role. Arc Hospice & Palliative Care is committed to transparency and equity in all compensation practices.
Final pay is determined based on factors such as experience, licensure, location, and business needs at the time of hire. Some roles may be eligible for additional compensation components consistent with Arc Hospice & Palliative Care practices.
Compensation
$65,000 - $80,000 USD
