Job Description Summary
The Hospice RN - After Hours provides responsive, compassionate clinical care to hospice patients during evenings, overnights, weekends, and holidays. This role handles urgent nursing visits, symptom management, and emotional support when primary teams are unavailable, ensuring continuity of care outside traditional hours.
Key Responsibilities
Respond to patient and family calls and perform in-home visits based on clinical necessity.
Assess symptoms and provide interventions for comfort, stability, or urgent need.
Communicate with on-call providers to secure medication orders and coordinate care.
Respond to patient death and complete nursing responsibilities including death pronouncement and post-mortem support when within scope.
Collaborate with emergency room providers and facility staff to support hospice-appropriate discharge plans, ensuring safe transitions and alignment with patient and family goals of care.
Conduct evaluation, consent, and admission visits when clinically indicated, ensuring timely initiation of hospice services after hours.
Ensure proper clinical handoff and documentation for the primary team.
Ensure accurate, real-time documentation of assessments, interventions, and communications.
Coordinate with the primary team to ensure continuity and safe transitions of care.
Maintain availability during assigned shifts in alignment with the on-call structure.
Ensure all documentation supports regulatory and billing compliance, including timeliness standards.
Escalate care needs to the medical director or covering provider as clinically indicated.
Performs other duties as required.
Qualifications
Graduate of an accredited nursing program; Associate’s degree or higher preferred.
Current RN license in practicing state or a valid multistate compact license.
Minimum one (1) year of hospice, palliative care, or home health experience preferred.
CPR certification and valid driver’s license required.
Strong clinical judgment, documentation accuracy, and communication skills.
Knowledge of hospice regulations and Medicare Conditions of Participation preferred.
Ability to work independently while collaborating across interdisciplinary teams.
Strong communication, critical thinking, and documentation skills.
Excellent communication, change management, and project execution skills are required.

The starting compensation for this position is shown below. Arc Hospice & Palliative Care is committed to transparency and equity in all compensation practices. The rate displayed reflects the starting hourly or salaried pay for the role and does not represent a range.
Final pay is determined based on factors such as experience, qualifications, location, and business needs at the time of hire. Some roles may be eligible for additional compensation components consistent with Arc Hospice & Palliative Care practices.
