Job Description Summary

The Nurse Navigator provides remote telephonic clinical assessment, support, and operational oversight for hospice patients, families, and after-hours clinical teams. This role serves as a central clinical command point during non-business hours, ensuring continuity of care, timely response to patient needs, and adherence to Arc Hospice clinical workflows. The Nurse Navigator pushes clinical workflow forward to maintain seamless operations after hours and deploys field clinicians in alignment with the Arc Hospice Triage SOP.

Key Responsibilities

· Receive and manage incoming triage calls with professionalism, clinical accuracy, and adherence to Arc Hospice standards.

· Conduct comprehensive telephonic assessments to determine patient acuity, symptom severity, and required interventions.

· Push clinical workflow forward after hours to maintain operational continuity, ensure timely follow-through, and support ongoing care plans.

· Deploy field nurses and clinicians based on the Arc Hospice Triage SOP, ensuring appropriate response levels and timely in-person care when needed.

· Provide clinical guidance, emotional support, and education to patients, families, and caregivers.

· Document all call encounters clearly, accurately, and in real time within the electronic medical record.

· Maintain HIPAA compliance and ensure secure, uninterrupted access to required systems while working remotely.

· Collaborate with interdisciplinary team members to support patient care plans and ensure continuity across shifts.

· Communicate promptly with clinical leadership and on-call providers regarding complex, urgent, or escalated situations.

· Identify and escalate safety concerns, care gaps, or urgent clinical issues to the Regional Director of Clinical Operations or on-call provider as appropriate.

· Perform other duties as assigned to support Arc Hospice operations and patient care.

Qualifications

· Graduate of an accredited nursing program; Associate’s degree or higher preferred.

· Current RN license in good standing with the ability to practice in all states of operation, either through individual state licensure or a valid multistate compact license.

· Minimum one (1) year of hospice, palliative care, or home health experience preferred.

· CPR certification and valid driver’s license required.

· Strong clinical judgment, critical thinking, and documentation accuracy.

· Ability to work independently while collaborating effectively across interdisciplinary teams.

· Excellent communication skills, including the ability to provide calm, clear guidance during high-stress situations.

Physical Requirements / Working Conditions

This position is fully remote and requires uninterrupted access to secure internet and a quiet, professional environment. The role involves extended periods of screen time, phone

communication, and documentation during assigned shifts. May occasionally support transitional communications across teams and time zones.
