We are looking for a Transition Coordinator to play a vital role to successfully transitioning new individuals into Aspire Living & Learning. S/he is integral part of the Referral and Intake process and facilitates a smooth transition for individuals into Aspires’ Adult Services. The Transition Coordinator works in conjunction with the state operations team to develop new programs and assists Shared Living Coordinators and Program Managers with setting up new programs. The Transition Coordinator is adept at positively interfacing with external agencies, families, and funders.
