Aspire Living & Learning is currently seeking a Community Companion Home Coordinator (CCH) to provide leadership, oversight, and hands-on support to Home Care Providers, ensuring the health, safety, and overall well-being of individuals in Shared Living arrangements. This role involves a high level of travel throughout the state of Connecticut—visiting homes, meeting with providers, and connecting with individuals in their communities. The CCH Coordinator builds strong, trusting relationships with providers and the individuals they support, ensuring each person has opportunities for personal growth, meaningful community connections, and a high quality of life. You will collaborate with Aspire’s management team, state agencies, family members, medical professionals, and other stakeholders to create supportive, inclusive environments where individuals can flourish.
Our Ideal Candidate:
- Bachelor’s degree or equivalent professional experience.
- Prior experience supporting individuals with disabilities is strongly preferred.
- Reliable transportation, a valid driver’s license, and willingness to travel.
- Strong communication skills, empathy, and enthusiasm for making a difference.
- Basic computer skills (experience with Therap is a plus!).
