BAYADA HOME HEALTH CARE is a nonprofit organization dedicated to compassion, excellence, and reliability. Our Assistive Care State Programs (ACSP) practice provides personal care and companionship for adults and seniors who need support with activities of daily living (ADLs), including self-care and household support services, primarily on an ongoing shift basis, often funded through state-sponsored programs and waivers.

As a Clinical Manager – Assistive Care State Programs, you will lead and support a team of caregivers and field staff, ensuring high-quality, compliant care for clients served through New Jersey state programs. You’ll play a key role in clinical oversight, EVV-driven documentation, and partnership with families and payors while helping clients safely remain in their homes. This office provides best-in-class care throughout Essex County and surrounding area

What You’ll Do

  • Lead, mentor, and support a team of HHAs, CHHAs, and CNAs providing services under New Jersey state-funded programs.
  • Complete and oversee admission assessments, care plan development, and ongoing clinical review for ACSP clients.
  • Ensure compliance with BAYADA policies, state program regulations, and EVV requirements (e.g., accurate visit verification and documentation).
  • Conduct supervisory visits, home assessments, competencies, and skills validations for new and tenured staff.
  • Partner closely with families, case managers, state agencies, and referral sources to support safe, client-centered care and strong outcomes.
  • Collaborate with office leadership on quality assurance, client experience, and performance improvement initiatives.
  • Participate in new hire orientation and ongoing education related to ACSP programs, clinical standards, and documentation.

What You Bring

Required:

  • Registered Nurse (RN), New Jersey license in good standing.
  • Bachelor’s degree in nursing or related field or equivalent clinical leadership experience.
  • 2+ years of RN experience in home care, community health, long-term care, or related setting.
  • Experience with supervision, mentoring, or leading clinical or caregiver teams.
  • Strong organizational, communication, and problem-solving skills, with attention to regulatory detail and documentation.

Preferred:

  • Experience in assistive care / personal care services, home care, or Medicaid/state program work.
  • Knowledge of New Jersey state waiver programs (e.g., CAP, JACC or similar) and payor requirements.
  • Bilingual (Spanish/French/Creole/Russian) a plus, to better serve the diverse communities of Essex County.

Schedule & Work Environment

  • Part-time, exempt role based in Bloomfield, NJ. Flexible daytime hours and no weekends
  • Combination of office-based work and community-based visits within Essex County and nearby areas.
