We're hiring an Advanced Practice Provider / Nurse Practitioner for our Biofourmis business entity, who currently licensed in the state of California, Florida, and New York (or at least 2 of those states) with a minimum of 2 years of specialized cardiology experience in the ambulatory or inpatient setting. In this role, you'll lead comprehensive care by managing E&M and TCM patient visits, coordinating in-home nursing orders, and facilitating seamless handoff calls to hospitalists. Through virtual video visits, you'll assess both acute and non-acute patients, document assessments in our EMR, and ensure all program guidelines are met. You'll also monitor/respond to any escalated alerts, promptly addressing vital changes, symptoms and health shifts to deliver proactive, high quality care.
The ideal candidate will work 11am-7pm EST.
Must be licensed in California, Florida, and New York (or at least 2 of these states)
Responsibilities:
- Virtually evaluate patients being enrolled into the different care programs and assist in creating a care plan based on patient specific needs and predesignated protocols
- Perform virtual video visits, assess acute and non-acute clinical problems
- Document visits in EMR according to program guidelines using all available patient data sources and bills to payors
- Respond in a timely fashion to alerts escalated from the virtual care team related to patients’ deviations in vitals and physiologic data; symptoms; weight changes
- Independently manage changes in clinical status that are within the APP’s scope of practice (e.g. dose change in diuretic after 3 pounds of weight gain over 24 hours)
- Escalate out of scope clinical changes to lead MD
- Liaise with the virtual care team to ensure proper medications, labs and virtual appointments are ordered and communicated to the patient
- Serve as the point of contact for each patient’s primary care team, relaying information to them and tailoring this communication to individual provider needs
- Provide education to patients about healthy behaviors and self-management
- Provide peer-to-peer education
Qualifications:
- A Board-Certified Nurse Practitioner or Physician Assistant with a minimum of 2 years of specialized cardiology experience in the ambulatory or inpatient setting
- Current active and unencumbered license to practice in California, Florida, and New York
- Ability to work 11am-7pm EST
- Previous telehealth experience is a plus
- Willingness to work in a changing, dynamic environment with a growing company
- Motivated to provide health education and have a grounding in health promotion techniques
- Able to work with healthcare providers at all levels and non-clinical personnel
- Proficiency with Mac, Windows-based computer applications, email and web-based skills.
- Ability to learn and navigate multiple EMRs
- Must be able to work from home with a HIPPA compliant environment
