Schedule:

Varied Per Diem Hours Occurring Monday Through Friday Between 8:30am Eastern Standard Time To 5pm Eastern Standard Time. Occasional Evening Hours May Be Required.

 

Location & Travel:

This role is associated to our DeWitt, NY location. It is anticipated that an incumbent in this role will travel within a two (2)-hour radius of the metro-area noted in this posting, as needed, to provide applicable patient care. Work location is subject to change based on business needs.

 

Job Description:

Performs specified patient care activities and related services necessary in quality care of the home nutritional support and intravenous therapy patient.

QUALIFICATION REQUIREMENTS

  • Graduate of a N.L.N. accredited school of nursing required
  • Current license as a Registered Nurse in the state in which you are currently practicing required
  • Minimum of one (1) year of experience in IV therapy with a strong background in nutritional support and/or immunoglobulin therapy required
    1. In lieu of above experience, two (2) years of recent experience in an acute care setting or apprentice for a period of 3 months for all home visits with an experienced R.N. may be accepted
  • Must possess or acquire a current CPR certificate within the first 30 days of employment
  • Must possess good technical skills and strong knowledge base in clinical nutrition and IV therapy, and must be able to function independently within a team framework
  • Must possess excellent communication and organizational skills
  • Must be dependable in attendance and job performance
  • Must have valid driver’s license in state of employment

QUALIFICATIONS PREFERRED

  • Experience in home infusion nursing preferred
  • Participation and membership in professional societies preferred

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Maintains standards and policies set forth by BioMatrix Specialty Infusion Pharmacy.
  • Performs initial patient evaluation as it pertains to nursing care in accordance with BioMatrix Specialty Infusion Pharmacy policy and procedures.
  • Assists referring physician and/or institution in planning and implementing patient care in order to insure a smooth transition from hospital to home.
  • Provides the patient/significant other with information regarding the concept of home infusion therapy and the impact of such therapies.
  • Performs patient training in self-management of infusion therapies including the provision of necessary teaching materials.
  • Conducts patient visits in the home/clinic to perform the following:
  • Physical assessment
  • Nutritional assessment
  • Therapeutic monitoring to evaluate and manage potential mechanical, infectious, and metabolic complications.
  • Peripheral IV starts/re-starts
  • Obtaining labs via peripheral veni-puncture or central venous access device.
  • Assessing patient's ability to perform procedures and instruction of specific procedures.
  • Performing therapy specific nursing procedures (i.e., port access, CVC dressing changes, etc.)
  • Product delivery
  • Maintains thorough and appropriate documentation in the patient medical records of all visits and procedures performed as well as phone contacts with the patient, physician, or other members of the health care team.
  • Participates in patient care conferences.
  • Assists Director of Nursing in conducting special projects.
  • Participates in the agency Performance Improvement Program and Patient Safety Committee.
  • Actively participates in and follows agency Infection Control Program.
  • Knowledgeable regarding and required to respond and participate in the agency Emergency Preparedness Program.
  • Communicates with staff in a pleasant, courteous, and professional manner.
  • Works well within a team framework as well as independently.
  • Demonstrates compliance with agency policies related to patient rights and customer satisfaction.
  • Communicates with patients and customers in a pleasant, courteous, and professional manner.
  • Coordinates all services and equipment patients may require at home in addition to IV therapy (I.E., DME equipment).
  • Communicates with patient's physician, verbally or in writing, on a regular basis regarding patient's progress.
  • Administers intravenous solutions and IV/IM/SC medications as ordered by the physician and monitors for pharmacological effects and toxicities.
  • Maintains proficiency in skills and theory of nutritional support and IV therapy by reviewing literature, attending in-services, seminars and conferences.
  • Registered Nurses are responsible for case management of all cases involving nursing and therapy care.
  • Responsible for the clinical record of each patient receiving nursing care
  • Assure that progress reports are made to the physician for patients under medical care when the patient’s condition changes or there are deviations from the plan of treatment.
  • Performs other duties as assigned by BioMatrix Specialty Infusion Pharmacy Management.
  • Complete all training & orientation with new RN’s. Provide ongoing training to nursing staff as needed. 
  • Participation, knowledge, training and reporting in the agency Corporate Compliance program. 
  • Participates in training with new Registered Nurses including documentation, completion of tasks in the home environment, communication with multi-disciplinary team.

  • Become and remain proficient is all programs necessary for execution of position.
