Currently, we provide virtual and in-person mental health services, including comprehensive evaluations, medication- management and therapy, across Pennsylvania and Virginia with plans to expand to new markets in 2025. 

Position Summary

The Care Navigator (part time), serves as the experienced care specialist for Blackbird Health patients.  As such, they are primarily responsible for engaging and supporting patients who have begun their care journey with a Blackbird Provider and support families in creating a shared understanding of the precision health care model that Blackbird Provides. Care Navigators are seen as a trusted partner to our patients and providers and as such navigate all established patient care and support a positive experience from the point of intake through to the engagement of treatment services

This is a part time remote position working 20 hours per week, Monday-Friday 4pm-8pm

How you’ll make an impact:

• Field inbound calls from patients ensuring their questions are answered in a timely manner, including directing calls or routing messages to appropriate team members
• You strive to provide outstanding customer service and promote and positive reputation for the organization 
• You act as a role model by displaying enthusiasm, positive problem solving and commitment to help both co-workers and the organization reach its goals
• Schedule and confirm various established patient appointments, including sending reminders and handling rescheduling and cancellation requests
• Schedule and confirm psychotherapy appointments to ensure therapist are scheduled to their full capacity expectations
• Navigate all established patient care and support a positive care experience from the point of intake through to the completion of treatment services by ensuring patients care questions are answered and they connect with their provider in a timely manner
• Adhere to confidentiality and data protection standards, including HIPAA regulations, to ensure patient privacy and secure handling of sensitive information.  
• Ethical and Legal Compliance - Performs all duties to the highest ethical standard and complies with all organizational policies regarding ethical business practices and standards of patient care. You comply with all standards established by agencies accrediting the organization 
• Attend team meetings to support organizational success and participate in ongoing training required by organization to ensure high quality services are delivered
• Independently problem solve and organize time to consistently achieve productivity and quality standards for role
• Use new technologies as directed by leadership (e.g., Luma communications platform, EHR, internal tracking systems)
• Virtual work model for hours of 4pm-8pm Monday through Friday with the option to add weekend hours

The basics you’ll need:

• Certified Medical Assistant or degree in Healthcare Administration or related field
• Minimum 2 years of experience working in a medical office or healthcare setting 
• Computer Skills - Must be PC literate with proficiency typing, navigating email, using calendar and word processing
• Experience working in an electronic health record or patient engagement platform required
• Experience working in mental health or psychotherapy office setting preferred
• Telephonic outreach experience
