Position Description
Bluebird Kids Health is seeking a dynamic Care Coordinator for our CARES Center (Center for Access to Resources, Education, and Support). The ideal candidate is a self-starter with strong interpersonal skills, experience in a medical receptionist role, and a proven ability to deliver results in a fast-paced, growth-oriented environment.
What You’ll Do
- Manage provider’s Clinical Inbox, including but not limited to Clinical Documents, Admin/Phone messages and Unknown Documents.
- Answer live calls on the referral line and respond to voicemail messages in a timely manner. Schedule follow up ADD/ADHD, Behavioral Consults, and Well Child Check-ups as needed. Manage all requests for mental health services and referrals. Manage requests for prescription refills. Verify patient demographics during every patient interaction/communication.
- Provide clear, thorough and accurate documentation of all interactions with patients and other individuals on behalf of patients in the patient’s EMR.
- Follow organizational guidelines regarding the use of EMR in compliance with HIPAA and patient confidentiality standards.
- Coordinate referrals for patients going outside the medical home for care, including STAT specialist appointments and procedures such as MRI, CT, U/S, and X-rays. Process necessary authorizations and follow up appointments.
- Manage coordination of chronic patients.
- Provide 504 letters as needed.
- Assist with scanning documents to patients’ charts.
- Maintain supply of specialists’ business cards for distribution to patients.
- Monitor appointment ticklers to proactively schedule appointments.
- Assist patients requiring Mental Health services.
- Perform other duties as assigned.
What You’ll Need
- 2- 4 years in a comparable role, or pediatric front office/medical administrative role.
- Experience in healthcare, social services, or childhood education, and familiarity with services available in South Florida
