A proud Equal Employment Opportunity and Affirmative Action employer, we consider all applicants without regard to any status protected under applicable law and we strive to employ qualified diverse talent. We encourage you to share information about yourself by answering the questions below, to help us comply with federal government reporting and recordkeeping requirements, evolve our ED&I initiatives, and measure the effectiveness of our efforts. Please review the Government Ethnicity and Race Definitions when answering those questions below.

Although all sections below require a response, you can always select "I don't wish to answer."  Your responses will be kept confidential and used only in a manner consistent with applicable law and will never be used against you. Thank you for sharing more about yourself and contributing to our affirmative action and diversity efforts.

Voluntary Self-Identification of Protected Veteran Status

Blueprint Medicines is a government contractor subject to the Vietnam Era Veterans' Readjustment Assistance Act of 1974, as amended by the Jobs for Veterans Act of 2002, 38 U.S.C. 4212 (VEVRAA). As such, we take affirmative action to employ and advance in employment protected veterans, including by reaching out to veterans' groups for qualified applicants and periodically reviewing our processes to ensure there are no unintended barriers. To help us measure the effectiveness of our outreach and recruitment efforts of veterans, we ask you to share if you are a veteran covered by VEVRAA.  Answering is completely voluntary, but we hope you will do so, and your response will be used and kept confidential consistent with VEVRAA, and will not be used against you in any way. Please review these Protected Veteran Descriptions and check the appropriate box further below as to whether you belong to any of these government-defined categories.

Voluntary Self-Identification of Disability

Form CC-305 OMB                                                                        Control Number 1250-0005
Page 1 of 1                                                                                                 Expires 04/30/2026

Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
