Nurse Practitioner / Physician Assistant - Primary Care (Group Home Population)

Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing in Group Homes in the northeast metro of Minneapolis/St. Paul.
Position Overview:
- Provide on-site, team-oriented primary care to patients within the group home setting living communities
- Part-time providers care for patients weekdays; tailored to part-time desires and needs
- Manage a panel of patients autonomously seeing approximately 8-14 patients per/day
- Ability to design and manage your schedule
- Develop and implement care plans appropriate to patients and their desires
- Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers
- Collaborate with community partners within home care, hospice, and other resources as needed
Qualifications:
- Graduate of NP program or PA program from an accredited school
- Must have and maintain a valid Minnesota License
- Professional clinical experience in primary care
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
- Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required
- Demonstrated compatibility with Bluestone’s mission and practiticing philosophies
- Travel throughout Twin Cities Metro required
Preferred Qualifications:
- 2+ years of clinical experience and/or group home experience in another role
