Medical Director/Primary Care Physician

Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

You will drive our value-based care model forward, providing expert mentorship to a dedicated team of APPs while ensuring clinical excellence for residents in assisted living and memory care environments. Key Responsibilities
Direct Patient Care
- Primary Care Delivery: Serve as the Primary Care Provider (PCP) for a dedicated panel of patients residing in assisted living and memory care facilities.
- Clinical Management: Provide comprehensive chronic and acute care management, tailoring treatment plans to the unique needs of geriatric and complex populations.
- Collaborative Care: Actively partner and communicate with assisted living staff to ensure seamless care coordination and adherence to practice standards.
Medical Directorship & Population Health
- Value-Based Care Leadership: Partner with a team of APPs to drive population health management initiatives, specifically focusing on the success of Value-Based Care programs.
- Mentorship: Provide clinical and operational mentorship to APPs, guiding them in the management of complex patients and efficient practice management.
- Strategic Contribution: Participate in key committees and task forces to lead process improvement, quality assurance, and policy development.
Regulatory & Clinical Support
- Compliance Visits: Conduct telehealth visits to satisfy state regulations and the specific regulatory requirements of various healthcare programs.
- Acute & Complex Coverage: Provide high-level clinical support and telehealth coverage for complex patient cases or urgent acute needs across the broader clinical team.
Qualifications:
- Medical License: Must be licensed or willing to become licensed in the state of Florida.
- Board certification: ABMS or AOA
- Experience: Proven experience in clinical practice, geriatrics and chronic disease management preferred, with a strong background in leadership and mentorship.
- Skills: Excellent communication and collaboration skills, with the ability to lead and support a diverse team of APPs.
- Commitment: A dedication to upholding high standards of clinical excellence and a passion for improving patient outcomes.
