Contract Care Coordinator Assessor, LSW or RN (Part-time or Full-time)

Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview:
Join our team as a Contract Care Coordinator Assessor where you will work from home, but also be required to travel about 30% of the time to meet with members in Hennepin County, MN.
Schedule: Looking for part-time OR full-time position contractors (please submit work availability with online application). Flexible timeframe of work within regular business hours.
Location: Approximately 70% work from home & 30% in-person travel to meet with members in Hennepin County, MN (Mainly throughout Minneapolis and St. Paul, MN with mileage is reimbursed per company and IRS guidelines).
Pay Range: Paid per assessment - Averages $35 per hour (hourly amount within range is based on experience).
Duration of Assignment: ASAP thru September/October 2026 with the potential of Temp to Full-time opportunities available.
Benefits: Not benefits eligible as a contract employee.
Responsibilities:
As a Contract Care Coordinator Assessor, you will complete assessments within your assigned geographic area - Hennepin County, MN.
This includes:
- Contacting new enrollees to complete the initial assessment. Contractors working full time hours will complete approximately 8 assessments per week.
- Reviewing current health needs, identifying goals, and developing individualized care plans and transitions of care
- Helping connect members with resources and services
- Completing required documentation
Qualifications:
Education/Certification/Experience
- MNChoices experience highly preferred (paid training available)
- MN Licensed Social Worker (new grads encouraged to apply)
- OR Current Minnesota Registered Nurse license
- One or more years of experience working with the geriatric or certified disabled population in case management/care coordination, Home Care, Group Homes, Nursing Homes, TCU or Assisted Living settings preferred
- Must have a valid driver’s license
