Nurse Practitioner or Physician Assistant – Dementia-Focused Care
Brigade Health | Full-Time | $100,000 – $170,000 (TCC), DOE

About the Role
We’re hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to support our dementia care program. This hybrid role combines clinic-based and telehealth visits, with opportunities to expand into senior living communities. You’ll conduct cognitive evaluations, manage behavioral and chronic conditions, and educate families navigating dementia.
What You’ll Do
- Conduct 45-minute cognitive assessments (CPT 99483)
- Deliver dementia-focused care via clinic and telehealth
- Coordinate with PCPs and care partners
- Manage follow-up visits and documentation
- Support senior living expansion initiatives
What We Offer
- Compensation: $100,000 – $170,000 total cash compensation (base + incentive)
- base salary, dependent on experience and qualifications
- Schedule: Monday–Friday, 8 AM–5 PM | No call or weekends

What You Bring
- Active NP or PA license (in good standing)
