This position is ideally structured as a PRN role, offering flexibility for an experienced clinician looking to make a meaningful impact. • Collaborate with providers, nurses, and memory care specialists to develop and update patient care plans.
  • Educate families on disease progression, caregiver strategies, safety planning, and available resources.
  • Support care transitions and connect patients with community programs, entitlement resources, and support services.
  • Assist with home safety evaluations and recommendations to support independent living.
  • Deliver services in patients' residences and/or through telehealth based on individual needs.
  • Maintain timely, accurate, and compliant clinical documentation.

  • Active Texas Licensed Clinical Social Worker (LCSW) license.
  • 3-5 years of experience in geriatric behavioral health, senior care, or related clinical settings.
  • Strong understanding of Alzheimer's disease, dementia, and the impact of cognitive decline on patients and family systems.
  • Experience providing counseling, psychosocial assessments, and care coordination.
  • Comfortable working independently while collaborating with an interdisciplinary team.
  • Ability to provide services in person, remotely, or through a hybrid model.
  • Successful completion of required background screening and health clearances.
Why Join Us?
  • Flexible PRN schedule with autonomy and work-life balance.
  • Opportunity for full-time employment for the right candidate.
  • Telehealth-friendly environment with local operational support.
  • Collaborative interdisciplinary care team.
  • Meaningful work that improves the lives of seniors and their families.
  • Opportunity to help patients remain safe, supported, and independent in their homes and communities.
If you are passionate about serving older adults and helping families navigate the challenges of aging, cognitive decline, and caregiving, we encourage you to apply.
