We are seeking a Per diem Admissions provide information about services to potential agency patients and admits patients to agency's total care category of service in coordination with Clinical Team Manager(s). The RN will also, provide education about services to medical providers within assigned territory and meet immediate admission needs of patients

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

Screen and process inquiries for admission to By the Bay Health's total care category.
Determine patient's eligibility for hospice care.
Make telephone contact and/or assessment visits with potential hospice patients to provide information on agency services and/or assess patient's status.
Develop relationships, maintain regular contact, and resolve issues with referral sources including physicians, hospitals, nursing facilities, residential care facilities for the elderly, senior housing, and community service agencies to ensure ongoing quality of care and referrals
Determine patient's eligibility for hospice car
Admit patients, obtain informed consent(s) for service, inform patient/family of rights and responsibilities

Education/Experience:

One year experience as a professional nurse in the past three years or Bachelor’s degree in nursing from an accredited School of Nursing.
Minimum one year experience in a home health/ hospice setting.

Certificates, Licenses, Registrations:

Current license as a registered nurse in the State of California
Public Health Nursing certification highly desirable
HPNA certification preferred
Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car

Pay Range:
$58.00 - $72.00 hourly
