
Patient Care Support Services Coordinator

The Patient Care Support Services Coordinator supports the Patient Care Support Services Manager by coordinating completion of PCSS processes and procedures in compliance with organization policies and procedures and applicable laws and regulations.
Bi-lingual in Spanish, Cantonese, Mandarin a plus!!
Schedule:
Hybrid; Monday - Friday; 8:30am - 5:00pm; includes rotating weekends and holidays
Location:
Larkspur; occasional travel to offices in Sonoma and S.F.
Essential Duties & Responsibilities include (but are not limited to):
- Complete routine chart audits at time of admission, discharge, and death for reporting at Quality Meetings
- Use electronic reporting systems to obtain patient data as requested
- Serve as central contact person for clinical staff and managers regarding patient medical records
- Identify, develop, and implement opportunities for additional or improved work methods to ensure accordance with policy and agency mission and goals
Education/Experience:
- Associates Degree (AA); or equivalent combination of education and experience
- Minimum of one year experience performing clerical/administrative tasks in a leadership role, preferably in a health care
Pay Range:
$25.00 - $30.00 hourly
