
Per diem Nurse Practitioner
By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.
Schedule:
Days are flexible; 8:30am-5:00pm
Location:
Marin, Alameda, Contra Costa & San Mateo Counties
By the Bay Health, Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Hybrid role with visits in home, facility, and inpatient settings

Travel within service region required; valid driver’s license and reliable transportation needed

Some evenings, weekends, and on-call coverage required

Deliver bedside assessments, pain and symptom management, and goals-of-care conversations for patients with serious or life-limiting illness

Conduct hospice face-to-face visits and document eligibility in compliance with CMS regulations

Collaborate with the interdisciplinary team to develop and update care plans

Educate patients, families, caregivers, and community providers on hospice and palliative care

Ensure accurate and timely documentation to support quality care and regulatory requirements
Education/Experience:
- Bachelor’s degree and graduate of accredited Nurse Practitioner Program or Master’s in clinical field of nursing or clinical field related to nursing
- Experience in an acute care, home health, palliative or hospice setting preferred
Certificates, Licenses, Registrations:
- Advanced practice Registered Nurse with active Nurse Practitioner license
- Public Health Nursing certification is highly desirable
- HPNA certification preferred
Pay Range:
$61.25 - $76.60
