
Palliative Care Manager

The Palliative Care Manager is a registered nurse leadership role responsible for the management, oversight, and coordination of clinical care and services for adult palliative care patients and their families. This role ensures delivery of high-quality, compliant, and patient-centered nursing care in collaboration with the interdisciplinary team, in accordance with organizational policies, professional nursing standards, and applicable laws and regulations.

Monday - Friday; 8:30am - 5:00pm
Hybrid - 3 days in office required

Essential Duties & Responsibilities include (but are not limited to):

Provide leadership and management oversight of the Adult Palliative Care Program under the direction of the Clinical Director

Collaborate with the Director of Pediatrics and Palliative Care to strategically grow the adult palliative care program throughout the San Francisco Bay Area

Participate in the development and implementation of clinical policies, procedures, protocols, and workflows aligned with program growth

Partner with contracted vendors, hospitals, and facilities to establish strong referral relationships and ensure high-quality patient care

Receive and coordinate new patient referrals and assign patients to appropriate clinical staff to ensure adequate coverage and continuity of care

Monitor clinical quality, outcomes, and regulatory compliance within the palliative care program
Supervisory Responsibilities:

Supervise non-exempt clinical staff

Provide clinical supervision to nursing team members

Provide administrative supervision to interdisciplinary team members as applicable

Participate in the Administrative On-Call rotation
Education/Experience:
-
Minimum of two years of registered nursing experience in home health, hospice, and/or palliative care within the past five years

Minimum of two years of supervisory or leadership experience preferred within the past seven years
Certificates, Licenses, Registrations:
- Current license as a registered professional nurse in the State of California, BSN preferred
- HPNA certification preferred
Pay Range:
$120K - $145K annually
