
Home Health LVN Intake & Scheduling Coordinator

The LVN Intake & Scheduling Coordinator serves as a central point of coordination for incoming Home Health referrals and provides licensed clinical support throughout the intake and scheduling process. This role is responsible for intake processing, medical record collection, insurance verification, EMR data entry, and scheduling of Start of Care (SOC) and subsequent visits. In addition, the LVN supports clinical intake functions, including preliminary clinical screening, prioritization, and coordination with Home Health clinical leadership to ensure safe, timely, and appropriate admissions.

Essential Duties & Responsibilities include (but are not limited to):
Intake & Referral Management
- Coordinate and process all new Home Health referrals from hospitals, physicians, SNFs, and community partners
- Gather and verify required referral documentation, including physician orders, clinical records, demographics, and insurance information
Scheduling & Start of Care Coordination
- Schedule Start of Care (SOC) visits in alignment with clinical priority, payer requirements, and staffing availability
- Coordinate ongoing visit scheduling in collaboration with clinical leadership and field staff
Clinical Intake Support (LVN Scope)
- Conduct initial clinical screening of referrals to support prioritization and safe admission planning
- Identify high-risk clinical factors (e.g., wounds, IV/infusion, recent hospitalization, fall risk, complex medication regimens) and escalate appropriately
Care Coordination & Communication
- Collaborate closely with Home Health clinical leadership, admission clinicians, provider relations, and billing teams
- Communicate effectively with physicians, hospitals, and facilities regarding referral status and admission timelines
Documentation & Compliance
- Maintain accurate, complete, and timely intake and scheduling documentation
- Ensure intake workflows align with Medicare, Medi-Cal, and managed care requirements
Education/Experience:
- Completion of a Licensed Vocational Nursing Education Program from an accredited school
- Minimum two years’ experience performing clerical duties, preferably in a healthcare setting
Certificates, Licenses, Registrations:
- Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners
Pay Range:
$36.50 - $45.65 hourly
