We're hiring a Licensed Practical/Vocational Nurse (LPN/LVN) to monitor a panel of remotely enrolled patients and move the ones who need clinical attention into care the same day. You will review incoming vitals and symptom data for patients with hypertension, heart failure, and diabetes, working alongside Cadence nurses and nurse practitioners to act on risk between visits. This role sits inside Cadence's Clinical Intelligence workflows, where alerts are surfaced continuously and licensed clinicians decide what happens next.

This is a part-time, remote position. The scheduled hours for this role are Saturday and Sunday from 2:00 PM to 11:00 PM Eastern time or 7:00 AM to 7:00 PM Eastern Time.

What You'll Do

  • Monitor incoming vital signs, symptoms, and health indicators for a panel of patients enrolled in Cadence remote monitoring.
  • Triage alerts and abnormal readings against established clinical criteria and identify patients who require immediate clinical intervention.
  • Contact patients with out-of-range readings to gather additional clinical information and confirm symptom, medication, and adherence context.
  • Coordinate with Cadence nurses and nurse practitioners to route flagged patients into the right intervention and close the loop on every follow-up.
  • Inspect AI-surfaced screening output and AI-drafted documentation for clinical accuracy before it reaches a provider, correcting anything the model gets wrong.
  • Document every patient interaction in the clinical record and build care summaries that give primary care providers and cardiologists a clear picture of what happened between visits.

What You Need

  • Active Licensed Practical/Vocational Nurse (LPN/LVN) license with compact licensure
  • Minimum 5 years of experience practicing as an LPN/LVN
  • Minimum 2 years of experience in a cardiology setting
  • Experience in remote patient monitoring or telemedicine, including EMR-based clinical documentation
  • Professional written and verbal communication skills for direct patient contact and provider-facing documentation
  • Fluency with AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) and the ability to speak credibly with providers and patients about where AI supports care delivery and where human judgment remains non-negotiable

Compensation

The anticipated OTE range for this role is $26 - $33 per hour, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
