
Centralized Intake Coordinator
This position is REMOTE, with flexibility to work in-office at branch locations if located in the area - and would be required to be available M-F 8:00am-5pm CST. In addition, NetSmart MyUnity experience is a requirement.

What are the job responsibilities?
In this role, you will serve as the first point of contact for all customers which includes patients, families, referral sources and co-workers. You will play a critical role in ensuring a smooth and timely transition from referral receipt to services. The ability to complete data entry accurately and timely directly into our EMR system is vital. In addition to intake coordinator responsibilities, this role will be an integral part of the Office Operations Team, contributing to system optimization, end-user support, and team training initiatives. You will also serve as a key resource for EMR related questions and workflow support to ensure efficiency, consistency, and system confidence company wide.
This position requires strong communication skills, prior Hospice and/or Palliative experience, and a solid working knowledge of the NetSmart MyUnity EMR system.
Key responsibilities include:
- Managing all aspects of the intake process
- Utilizing electronic medical records and the EMR system
- Collaborating with Clinical, Operations and Business Development teams
- Initial EMR support and providing on-going training and education
- Documentation and improvement of internal processes related to Intake
What do you need?
- High school diploma or equivalent required
- Proficiency in Netsmart MyUnity EMR required
- Strong knowledge of hospice/palliative eligibility criteria, CMS regulations and insurance entry processes
