Health Partnerships Manager
Position Summary:
We are seeking a dynamic and experienced Health Partnerships Manager to lead the implementation of multiple grant-funded programs that aim to improve community health outcomes through collaborations among food banks, healthcare organizations, and other partners.
This pivotal role requires a strategic thinker with broad expertise in community and population health, deep knowledge of one or more specialty areas, such as healthcare, agriculture, economic development, and/or food systems, and a proven track record of managing complex programs from conception to evaluation. The Manager will be responsible for fostering strong stakeholder relationships, driving impactful community engagement, and using data to measure success and inform future strategies. Leads and supervises 1-2 team members.
Key Responsibilities:
- Pilot Initiative Implementation: Lead the end-to-end implementation of new community health pilot programs, including planning, resource allocation, and project management to ensure successful launch, operation, and outcomes.
- Stakeholder Collaboration: Build and maintain strong, collaborative relationships with diverse stakeholders, including community-based organizations, agricultural workers, managed care organizations, healthcare providers, local government agencies, advocates, and food banks.
- Evaluation and Reporting: Design and implement robust program evaluation frameworks using key performance indicators (KPIs). Collect, analyze, and interpret data to assess program effectiveness, identify areas for improvement, and generate reports for leadership and funders.
- Community Engagement: Develop and execute effective community engagement strategies to ensure programs are community-driven and culturally relevant and address the specific needs of priority populations.
- Sector Expertise: Leverage expertise in the healthcare and/or agriculture sectors to identify opportunities to integrate health and food access programming and create sustainable solutions that address social determinants of health.
- Budget Management: Oversee project budgets, ensuring all activities are conducted within allocated resources and financial guidelines.
Qualifications:
- Experience:
- Minimum of 5-7 years of experience in program management, community health, public health, or a related field.
- Demonstrated experience in implementing and managing pilot programs or new initiatives.
- Expertise in healthcare, agriculture, food systems, economic development, labor, and/or other relevant sectors: Proven professional experience working within the healthcare system (e.g., hospitals, clinics, public health departments); experience working with managed care organizations, especially in patient experience, care navigation, or technology solutions, a plus.
- Skills:
- Strong project management skills, capable of managing multiple projects simultaneously in a fast-paced environment.
- Excellent written and verbal communication skills, with the ability to present complex information clearly to diverse audiences.
- Proficiency in data collection, analysis, and program evaluation methodologies.
- Strong interpersonal skills and a proven ability to work collaboratively and build consensus among various stakeholders.
- Education:
- Bachelor’s degree in Public Health, Healthcare Administration, Nonprofit Management, Social Work, or a related field required. Master’s degree preferred.
Compensation: Salary offers are determined based on final candidate qualifications and experience. The budgeted annual salary range that CAFB expects to pay for this position is $88,862-$110,000
