Lead Care Navigator, Remote US

Remote, US

About Carewell

Carewell is a category-defining business dedicated to providing trusted caregiving solutions and support for individuals and families. Through Carewell Family Services, we extend our commitment beyond products to person-centered navigation, care coordination, and advocacy services that address both medical and social needs. Our approach emphasizes compliance, scalability, and high-quality patient experiences while working in close partnership with clinicians and community resources to support better outcomes.

Our Patient Advocates are the heart of this work.

About the Role

This is a rare opportunity to be the first clinical hire at a growing care navigation program. You won't just build the program — you'll live it. In this dual role, you'll carry an active caseload of patients while working alongside leadership to design the infrastructure, workflows, and standards that will define how we deliver care at scale. As the program grows, so will you — with the potential to grow into a leadership role overseeing a multidisciplinary team of advocates, CHWs, and clinical staff.

If you're energized by doing the work and building the system around it, this role was written for you.

What You'll Do

Clinical & Patient-Facing

  • Deliver hands-on care navigation services to a diverse patient population
  • Conduct SDOH screenings and connect patients to community resources, benefits, and support services
  • Serve as a consistent advocate for patients navigating complex health and social systems
  • Build trusted relationships with patients, families, and care teams
  • Identify gaps in care and escalate or intervene appropriately
  • Appropriately document all patient interactions in all relevant systems (not solely in a Electronic Health Record (EHR)
  • Log start time, stop time, and duration for each interaction to support accurate monthly minute aggregation

Program Building

  • Partner directly with leadership to design and document care navigation workflows, SOPs, and standards of care
  • Help define program parameters, intake processes, and outcome metrics
  • Contribute to technology selection, implementation, and optimization
  • Develop training materials and onboarding frameworks in preparation for team growth
  • Provide real-time clinical insight to inform strategic decisions

Growth & Leadership (potential trajectory)

  • For the right candidate, there is potential to transition into a lead/supervisory role overseeing advocates, CHWs, LVNs, and other staff
  • Contribute to hiring, mentoring, and performance of future team members
  • Serve as a culture carrier and clinical role model as the team scales

Who You Are

  • Clinically grounded — LVN preferred; RN considered for candidates with demonstrated leadership potential and genuine appetite for direct patient work
  • Care navigation experience — 2–5 years minimum in care navigation, case management, care coordination, or a closely related patient-facing role
  • SDOH fluency — comfortable navigating social determinants and connecting patients to resources across complex systems
  • Builder mentality — you see the gap, you fill it, and you document how you did it so others can follow
  • Nimble and adaptive — you thrive in ambiguity and treat a fast-changing environment as an opportunity, not a stressor
  • Tech-forward — comfortable with care management platforms, EHRs, and digital tools; quick to learn new systems
  • Resilient problem-solver — you don't wait for perfect conditions; you find a way
  • Low ego, high output — equally comfortable doing the unsexy work and presenting to stakeholders

Nice to Have

  • Experience in a startup, pilot program, or ground-up initiative
  • Familiarity with value-based care or population health models
  • Bilingual (Spanish or other languages depending on your target population)
  • CHW (Community Health Worker) certification or cross-training
  • Experience with Motivational Interviewing or trauma-informed care frameworks

Why This Role

  • Ground-floor opportunity to shape a program from day one
  • Close partnership with leadership — your voice will matter
  • Competitive compensation with growth trajectory tied to program expansion
  • Meaningful, mission-driven work with visible patient impact

What We Offer

  • Competitive compensation
  • Health, Dental, and Vision insurance
  • Short-term Disability and Life Insurance (100% employer-sponsored)
  • Long-term Disability
  • Supplemental Life Insurance (employee-sponsored)
  • 401(k) Retirement Plan
  • 100% Remote
  • Generous paid time off and 6 paid holidays
  • Employee discount

💡 Tips for a Successful Interview with Carewell

We value your time and want to get the most out of our conversation. To help us understand your experience, please keep the following tips in mind:

1. Use the STAR Method

When we ask about your past experiences, we are looking for specific stories rather than general philosophies. We highly recommend framing your answers using the STAR method:

  • Situation: Set the scene (briefly).
  • Task: Describe what you were responsible for.
  • Action: Explain the specific steps you took.
  • Result: Share the outcome (with data or metrics if possible).

2. Be Specific, Not Theoretical

We want to hear about what you have actually done, not what you would do in an ideal world. Avoid phrases like "I usually..." or "In my opinion..." and instead start with "There was one time when I..."

3. Focus on Your Individual Contribution

While teamwork is important, we are hiring you. When describing projects, be sure to highlight your specific role and the impact you personally made, rather than just using "we."

4. Keep it Concise

We have a lot to cover! Aim for answers that are 2–3 minutes long. If we need more detail, we’ll be sure to ask follow-up questions.

5. KPIs / Metrics

Carewell is results driven. Come prepared to talk about the key metrics you were responsible for driving in similar roles. We’ll want to know how you directly measured, reported on, and improved those metrics.
