Care Coordinator I
We have multiple opportunities to join the Alliance as a Care Coordinator I in the Community Care Coordination Department within our Pediatric Complex Case Management teams.
WHAT YOU'LL BE RESPONSIBLE FOR
Reporting to our Pediatric Complex Case Management Supervisor, this position:
- Assists assigned team with the coordination of health care activities and/or health education activities for Alliance members
- Establishes and maintains effective working relationships with Provider offices, County departments and other community agencies related to care coordination for members, disease management, and/or health education
- Recommends and implements program improvements that strengthen member access and health outcomes

- Knowledge of:
- Bilingual (English/Spanish) desired, but not required
- The principles and practices of managed health care and health care systems
- Customer service
- Care coordination principles and practices
- The diverse needs of the Medi-Cal population
- Ability to:
- Competently navigate computer systems
- Use Microsoft Word and Excel, and Alliance computer systems
- Work independently with minimal supervision
- Verbally communicate the program mission, vision and roles
- Effectively, clearly and independently document, summarize and resolve member’s concerns and inquiries
- Conduct telephone interviews in a confidential and sensitive manner
- Identify, maintain, and protect sensitive HIPAA information (Personal Health Information) and following procedures to ensure the security of such information
- Education and Experience:
- High school diploma or equivalent
- Three years (or an Associate's degree in health, social services or a related field or Medical Assistant certification and a minimum of two years) of relevant experience in a health care setting interacting with members, patients and/or providers in meeting service needs; or a combination of education and relevant work experience may be qualifying
OTHER DETAILS
- While this position is connected to one of our Alliance offices, we are in hybrid remote/in-office work environment right now and we anticipate that the interview process will take place remotely.
- Our Alliance office locations have officially re-opened as of May 2, 2022 and while some employees may work in full-time telecommute schedules, attendance at quarterly company-wide events or department meetings will be expected.
- Based on the nature of work, this position may require onsite presence, which is dependent on business need. Details about this can be reviewed during the interview process.

The actual compensation for this role will be determined by our compensation philosophy, analysis of the selected candidate's qualifications (direct or transferrable experience related to the position, education or training), as well as other factors (internal equity, market factors, and geographic location).
Santa Cruz County Pay Range
$25.60 - $40.97 USD
Merced County Pay Range
$23.30 - $37.28 USD
Monterey County Pay Range
$25.60 - $40.97 USD
Mariposa County Pay Range
$23.30 - $37.28 USD

Available for all regular Alliance employees working more than 30 hours per week. Some benefits are available on a pro-rated basis for part-time employees. These benefits are unavailable to temporary employees while on an assignment with the Alliance.
