Part-Time Post Acute Nurse Practitioner in Brady, TX

We have an exciting opportunity for a Nurse Practitioner in Brady, TX

We are seeking a PT Nurse Practitioner to join Trumen Physicians and Associates.  Responsibilities include:

• Collaborating with the medical director in the management of patients
• Meeting regularly with medical director to discuss patients and collaborate on the plan of care
• Build and maintain a working relationship with facility administration
• Available Monday through Friday in the facility during normal working hours
• Effectively develop relationships with SNF Administrative and Clinical Teams
• Act as a liaison between the SNF and Trumen Physicians
• Ongoing support for the facility(s) quality improvement and patient safety programs as well as assistance in tracking and analyzing data trends
• Assist and Track Patients in Trumen Value Based Programs

Requirements Include:

• Must have a minimum of one year experience working as a Nurse Practitioner preferably in a Post-Acute Care setting.
